Are You Writing Specifications? Defining Requirements?

We have seen requirements specifications from many countries. Several of them have such major problems that a sub-optimal result is almost guaranteed. This is a disastrous outcome for such a significant national system. Major errors made in defining requirements include:

Assuming a bespoke software development process, with corresponding project phases and intellectual property implications. This can prevent the tendering of some of the best software available with superior functionality to what is envisaged in the requirements.
Specifying "how" rather than "what". This constricts potential suppliers to do things one way, even if they have better and more cost-effective solutions. This includes detailed data capture methods, data models, screen designs or process steps; especially when they are made mandatory. A bad example of this is defining the requirement to "scan arrival forms using OCR in a back office after arrival to capture additional data".
Unintentionally having specifications that are satisfied only by a single supplier. Specifications sometimes even include registered trademarks of a supplier! This is usually caused by limited research and insufficient market unerstanding.
